The prevalence of of delirium and dementia among persons discharged from acute care ranges from 4% - 27%. This is associated with increased:

  • Functional disability
  • Hospital LOS
  • Hospital readmissions
Course Learning Objectives: 

At the end of this webinar the participant will be able to: 

  • Develop an approach to early detection and treatment of delirium across the continuum from acute to long term care
  • Develop a patient/family inter professional protocol for delirium
  • Utilize an interprofessional team to diagnose and treat delirium
  • Decrease the incidence of delirium in patients returning to long term care
