The prevalence of of delirium and dementia among persons discharged from acute care ranges from 4% - 27%. This is associated with increased:
- Functional disability
- Hospital LOS
- Hospital readmissions
At the end of this webinar the participant will be able to:
- Develop an approach to early detection and treatment of delirium across the continuum from acute to long term care
- Develop a patient/family inter professional protocol for delirium
- Utilize an interprofessional team to diagnose and treat delirium
- Decrease the incidence of delirium in patients returning to long term care
